How do we get started?

As a component of the initial evaluation you are required to complete several forms prior to your first appointment including a New Patient Intake, Insurance/Demographics Information, a Treatment Consent/HIPPA form , Telehealth Consent, and a copy of Office Policies, & Crisis Resources.

If applicable, you may also be asked to complete a Controlled Substance Agreement and release of information (ROI) form.

Do you offer face to face appointments?

Currently, I am offering only telehealth services. In-person appointments may be available in the future, however with respect to current Covid-19 protocols, this option remains closed for the time being.

A HIPPA compliant platform is used for the entirety of the appointment to ensure confidentiality and privacy.

Please have access to a private space for telehealth appointments to ensure the confidentiality of any information we discuss and to maintain the integrity of the therapeutic relationship. A robust internet connection will be required connect for the appointment.

 

How do I reach you?

Please send messages through the agreed upon secure patient portal. I check messages on business days and you can expect a response within 48 hours.

Additionally, I may be required to share information related to insurance reimbursement and/or coordination of care with your other healthcare providers.

24 hour call is not available. I am not available for urgent or emergency concerns overnight, on weekends, or holidays.

Confidentiality

All information you share during an appointment is HIPPA protected and considered part of your medical record. I am a mandatory reporter, which means that I am required by law to report any suspected child or elder abuse, or risk of harming yourself or others.

Termination of Treatment

Termination of treatment can occur for a variety of reasons which may include but is not limited to:

  • Non-payment

  • Disruptive, hostile, or aggressive behavior

  • Frequent late cancellations or no-show appointments

  • Non-adherence or overt lack of participation with the agreed upon plan for treatment
