Insurance Billing

I am currently accepting Aetna, Regence/Blue Cross Blue Shield, and PacificSource/Reliant Behavioral Health
I suggest that patients verify coverage with their insurance provider prior to making an appointment to verify coverage.

I DO NOT CURRENTLY ACCEPT MEDICARE/MEDICAID

A “Superbill” can be provided for clients who wish to submit to insurance companies related to out-of-network reimbursement.

Payments are due at time of service, which includes any applicable co-pays or service fees

Out of Pocket Rates:

• Initial evaluation $450

• Follow up visits $300-$500 based on services provided.

Any requested letters are $25 per page

Medication Refills

Please submit refill requests at least 5 days in advance via the secure patient messaging portal. I will not respond to refill requests sent by voicemail, email or from your pharmacy.

Missed Appointments

Please cancel appointments at least 48 hours in advance, or a $150 fee will be assessed
